LPN Residential Services Specialist

ServiceNet is seeking a skilled LPN to join our team as a Residential Services Specialist. In this role, you will work closely with our developmentally disabled and brain injured clients in their residential programs, providing high-quality nursing care and advocating for their medical needs.
Key Responsibilities:
Monitor and coordinate healthcare services for participants
Institute appropriate follow-up and communicate medical issues to physicians, nurse practitioners, and other medical licensed providers
Document all nursing interventions and changes affecting the individual in appropriate areas
Maintain current medication and treatment orders according to MAP standards
Assist in training staff in personal care, positioning, feeding, and use of supportive devices
Accompany patients to medical appointments and Day Programs
Requirements:
High School Diploma / GED required
College Degree / Certificate preferred
Valid MA Nursing License
Valid driver's license and acceptable driving record
Physical ability to perform the requirements of the position
Good communication and computer skills
