Job Title: LPN - Developmental Brain Injury Services

We are seeking a skilled LPN to join our team at ServiceNet, a non-profit agency dedicated to supporting individuals with mental health challenges, developmental disabilities, brain injuries, and homelessness.

Job Summary:

The LPN will work in our residential programs, providing high-quality nursing care to our clients with developmental brain injuries. The successful candidate will be responsible for monitoring and coordinating healthcare services, implementing appropriate nursing interventions, and communicating with medical professionals and other staff members.

Responsibilities:
  • Monitor and coordinate healthcare services for participants, including medication management and treatment orders.
  • Implement appropriate nursing interventions, such as personal care, positioning, and feeding, to support the health and well-being of our clients.
  • Communicate effectively with medical professionals, Area Director, Nursing RN Supervisor, and other staff members to ensure seamless care coordination.
  • Document all nursing interventions and changes affecting the individual in appropriate areas, including daily logs, progress notes, and staff communication logs.
  • Assist in training staff in personal care, positioning, feeding, and use of supportive devices.
  • Accompany patients to medical appointments and Day Programs.
Qualifications:
  • High School Diploma or GED required.
  • College Degree or Certificate preferred.
  • Valid MA Nursing License.
  • Valid driver's license and acceptable driving record.
  • Physical ability to perform the requirements of the position.
  • Good communication and computer skills.
Benefits:
  • $68,640 - $79,040 per year, based on experience and education.
  • Additional sign-on bonus up to $5,000 (half received at time of hire, half received after 6 months of successful employment).
