The Psychosocial Care Coordinator is responsible for conducting thorough evaluations and formulating a comprehensive care strategy to meet the psychosocial needs of patients and their families. This role also involves the implementation, coordination, and assessment of program services while guiding families on available resources and information.

Essential Qualifications


• Master's Degree or higher from an accredited institution


• Licensed Clinical Social Worker (LCSW) in the State of Texas


• Minimum of 2 years' experience in social work within a healthcare environment, preferably with children and adolescents


• Valid Texas Driver License


• Current CPR Certification

Key Responsibilities


• Conducts comprehensive evaluations and formulates care plans to address the psychosocial needs of patients and their families.


• Provides direct patient support within the appropriate licensed discipline, as dictated by departmental requirements.


• Aids patients and families in obtaining community referrals and assists with applications for insurance programs, disability placards, accommodation, transportation, and other essential services.


• Engages in professional development training related to social work and human resources in the rehabilitation sector.


• Supplies information about external services to patients who do not qualify for in-house assistance.


• Acts as a liaison for Texas Medicaid's Medical Transportation Program to confirm the eligibility status of patients.


• Effectively communicates the social, psychological, and emotional needs of patients and families to the assigned care team.


• Consults promptly with the team upon identifying any concerning, unsafe, or significant changes in patient and family dynamics.


• Participates as a member of the external needs committee, which identifies necessary medical equipment for underinsured or uninsured patients.


• Manages pantry resources and tracks reporting in accordance with established requirements.


• Documents all social work-related follow-ups, paperwork, communications, and meetings in the electronic medical record (EMR).


• Handles incoming and outgoing faxes, ensuring records are scanned into the appropriate sections of the EMR.


• Conducts home visits as requested by the medical and therapy teams.


• Attends meetings as necessary based on family and social work needs.


• Safeguards confidential information in compliance with HIPAA regulations.


• Organizes informational sessions or services for families in collaboration with interdisciplinary teams and external organizations.


• Identifies departmental needs and contributes to the development of new programs.


• Performs additional duties, tasks, and special projects as assigned.
