Role Summary and Qualifications The Lung Transplant Care Coordinator plays a vital role in assessing the needs of lung patients and their families, providing education, and coordinating care throughout the transplantation process. This position involves clear communication with patients, families, and the Transplant Team, as well as other healthcare providers and payers.
Key Responsibilities:
- Monitor patients and their families throughout the transplantation journey, addressing physical, emotional, psychosocial, and financial needs as necessary. - Refer patients to appropriate members of the Lung Transplant team (such as chaplains, counselors, nutritionists, and financial advisors) and utilize community resources when needed.
- Organize the pre-admission assessments and visits for both patients and donors, collecting and reviewing relevant clinical information to present to the transplant physician, highlighting any abnormal results.
- Collaborate with healthcare providers to develop a comprehensive care plan, ensuring continuity of care across departments (both inpatient and outpatient) and with home health services.
- Facilitate the transition of patient care from the Lung Transplant Program back to the referring physician, overseeing long-term follow-up, including updates on patient status, coordinating follow-up appointments, and ensuring necessary post-discharge services are provided.
- Educate patients, families, and donors to enhance their understanding of the disease and the processes surrounding pre- and post-transplant care. Serve as a resource for the interdisciplinary team regarding transplant protocols to foster positive outcomes for lung transplant patients.
- Ensure timely communication with interdisciplinary team members, patients, families, and payers to maintain comprehensive documentation.
Required Qualifications:
- A Bachelor's degree in nursing is preferred. - A minimum of three years of recent clinical experience in Lung/ECMO/Transplant and familiarity with case management/utilization is preferred.
- This position requires full vaccination for COVID-19 in accordance with local, state, and federal regulations (unless a medical or religious exemption is granted).
Minimum License and Certifications: Must be licensed as a Registered Nurse in Texas.
