Hospice Care Assistant

As a Hospice Aide, you will work collaboratively with our interdisciplinary team to provide high-quality care to patients and their families. Your primary focus will be on delivering compassionate and personalized support, ensuring the comfort and dignity of our patients.
Key Responsibilities:- Provide personal care to patients, including bathing, dressing, and grooming, as per their individual care plans.
- Assist patients with activities of daily living, such as eating, toileting, and mobility, under the supervision of our registered nurses.
- Document patient care in our electronic health record, adhering to organizational and professional standards.
- Schedule and plan patient-centered visits, taking into account care plans developed by our registered nurses.
- Offer emotional support and comfort to patients and their families, promoting a sense of calm and well-being.
- Participate in team meetings and quality improvement initiatives, contributing to the continuous improvement of our care services.
- High school diploma or equivalent required.
- Meet the requirements for hospice aide qualifications under Medicare Conditions of Participation.
- Successful completion of in-house competency testing.
- At least one year of experience in a nursing home or home health agency preferred.
- Ability to understand written and verbal instructions, with strong interpersonal and communication skills.
- Must be flexible and able to cope with mental and emotional stress, demonstrating emotional stability.
- Valid driver's license, auto liability insurance, and reliable transportation required if driving is necessary.
Our Hospice Aides work in a fast-paced, dynamic environment, providing care to patients in various settings. You will be required to exert up to 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. You will also be exposed to hazards such as chemicals, infectious diseases, and atmospheric conditions like fumes, odors, dusts, mists, and gases.
