RN Hospice Care Specialist

At Amergis, we are seeking a compassionate and skilled RN Hospice Care Specialist to join our team. As a key member of our hospice care team, you will provide high-quality patient care and support to terminally ill patients and their families.
Responsibilities:- Assess patient needs and develop individualized care plans
- Provide emotional support and comfort to patients and their families
- Collaborate with interdisciplinary teams to coordinate patient care
- Monitor patient progress and adjust care plans as needed
- Document patient information and communicate with healthcare teams
- Current RN license for the state in which you practice
- Experience in hospice care or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Computer proficiency and ability to learn new software
