Substance Abuse Outreach Coordinator

Venice Family Clinic is a renowned healthcare provider dedicated to delivering high-quality care to individuals in need, regardless of their income, insurance, or immigration status. With over 500 staff members serving 45,000 people across a vast area spanning from the Santa Monica Mountains through the South Bay, we pride ourselves on our compassionate, dignified, and respectful approach to healthcare.
We have a network of clinics, Early Head Start centers, and mobile clinics, as well as an expansive street medicine program, to reach people experiencing homelessness. We strive to make a positive impact in the community through our Venice Family Clinic as a Substance Abuse Outreach Coordinator.
This role plays a critical part in providing linkage to care for clients utilizing our Harm Reduction Program, working under the guidance and direction of the Case Management Supervisor. The ideal candidate will possess in-depth knowledge of the healthcare system in Los Angeles and meet the following requirements:
- Bachelor's degree in social work plus one year of experience, or an equivalent combination of education and experience.
- Expertise in providing individualized HIV health education, HIV testing and counseling, Hepatitis C education, Hepatitis C testing and counseling, overdose prevention, medical and substance abuse referral, and abscess education.
- Knowledge of HIV/AIDS, including antibody testing, community resources, Hepatitis C, and STD knowledge, harm reduction, and syringe exchange.
- Proficiency in database management and data entry.
- Ability to complete required program documentation.
- Public speaking and leadership ability.
- Ability to work independently and as part of a group, and maintain cooperative working relationships.
- Ability to work effectively with people of diverse cultural and educational backgrounds and discuss sensitive matters in a non-judgmental manner.
