Our Home Care Assistants play a crucial role in our organization, delivering support to our patients in their residences and actively participating in the care planning process as integral members of our interdisciplinary team (IDT). We are committed to delivering exceptional care to our patients while fostering a positive work atmosphere for our staff.

Key Responsibilities:

  • Deliver hands-on personal care and assistance with daily living activities (ADLs) to WelbeHealth patients in their homes, including but not limited to bathing, grooming, dressing, toileting, and physical lifting during transfers.
  • Assist patients with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, shopping, etc.
  • Support with light meal preparation and feeding in patients' homes, ensuring they remain nourished and hydrated.
  • Facilitate safe transfers in and out of homes and serve as a medical escort during transportation and medical appointments as needed.

Job Qualifications:

  • High School Diploma or equivalent
  • Reliable transportation required
  • Bilingual in Spanish
