Role Overview: The Home Care Provider will offer personal assistance services to WelbeHealth participants within their residences.
- Full-time employment schedule
- Menlo Park, CA vicinity
- MANDATORY: Valid California Driver's License and auto insurance
- Willingness to work 40 hours weekly - No part-time options - No overnight shifts
Key Responsibilities:
- Deliver personal care and support with daily living activities (ADLs) to WelbeHealth participants, including bathing, dressing, grooming, toileting, and transferring.
- Assist participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
- Record and monitor participants' vital signs as directed.
- Help with meal preparation and feeding, ensuring participants receive proper nutrition and hydration.
- Facilitate safe transfers in and out of residences and serve as a medical escort when assigned.
- Support the Rehabilitation department by implementing a home exercise program as outlined in the care plan.
- Provide translation services as necessary, if qualified.
