Role Overview: The Home Care Provider will offer personal assistance services to WelbeHealth participants within their residences.

  • Full-time employment schedule

Key Responsibilities:

  • Deliver personal care and support with daily living activities (ADLs) to WelbeHealth participants, including bathing, dressing, grooming, toileting, and transferring.
  • Assist participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Record and monitor participants' vital signs as directed.
  • Help with meal preparation and feeding, ensuring participants receive proper nutrition and hydration.
  • Facilitate safe transfers in and out of residences and serve as a medical escort when assigned.
  • Support the Rehabilitation department by implementing a home exercise program as outlined in the care plan.
  • Provide translation services as necessary, if qualified.
