Clinical Pharmacist

Soliant Health is seeking a highly skilled Clinical Pharmacist to join our team in a hospital-based setting. As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to patients, documenting drug information, and adhering to departmental policies and procedures.
Key Responsibilities- Documenting and Reporting: Accurately document drug information questions, drug utilization evaluation activities, and adverse drug reactions.
- Patient Care: Understand and respond effectively to the unique needs of patients across various age groups, as demonstrated by effective documentation in progress notes and pharmacy care plans.
- Leadership and Guidance: Ensure adherence by pharmacy interns, residents, and technicians to departmental policies and procedures, as well as State and Federal laws and regulations.
- Formulary Management: Promote adherence to the approved Drug Formulary by the Pharmacy and Therapeutics Committee of the Medical Staff.
- Charging and Billing: Ensure proper charging documentation is completed correctly for services provided.
- Community Outreach: Promote and market the pharmacy department to the community, patients, hospital employees, and medical staff in an informal manner.
- Communication: Maintain a positive, direct, open communication line with members of the Medical Staff and their employees.
- Clinical Competency: Maintain clinical competency through successful completion of self-study modules and providing clinical services in practice.
- Teaching and Supervision: Provide teaching, direction, and supervision of pharmacy students, residents, and medical assistants working under your direction.
- Professional Development: Participate in and support professional organizations.
- Program Development: Take an active, participating role in the development and implementation of new programs, while supporting and facilitating existing programs.
- Competency Assessments: Complete assigned competency assessments.
- Additional Responsibilities: Complete additional responsibilities/projects when assigned by Director.
- Education: Completion of an advanced degree is preferred, but not required. Completion of an ASHP Accredited Residency and/or Fellowship is also preferred, but not required.
- Experience: Demonstrated experience in anticoagulation therapy, bridge therapy, and drug information is required.
