We are seeking a highly skilled and motivated Pharmacist to join our team at ResCare Premier Inc. as a Long-Term Care Specialist. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the safe and effective use of medications in our long-term care facilities.
Key Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Collaborate with the healthcare team to provide clinical and technical support, ensuring seamless communication and coordination of care.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Assist facility/agency staff with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
