Job Title: Forensic Clinical Case Coordinator
Job Summary:
We are seeking a highly skilled and compassionate Forensic Clinical Case Coordinator to join our team at the Center for Alternative. As a key member of our Nathaniel Community Success (NCS) program, you will play a vital role in providing clinical case management services to individuals with mental illness, including substance use problems.
Program Overview:
The NCS program is an innovative alternative-to-incarceration (ATI) program that serves individuals of all genders aged 16 and older with mental illness, including substance use problems. Our program operates in Manhattan and Brooklyn, leveraging the expertise of the Center for Alternative in providing jail and prison alternatives and mental health treatment options.
Key Responsibilities:
- Collaborate with the NCS team to collectively manage the team caseload and provide in-person services in the program office and in the community.
- Conduct onsite program groups and provide intensive case management services by communicating with community treatment providers, conducting site visits, and attending case conferences.
- Assess participants' immediate needs, legal circumstances, and engagement needs, and use Motivational Interviewing to identify risk factors for criminal recidivism.
- Act as an advocate and liaison for participants in securing access to community services, housing, income support, and natural supports.
- Complete charting using database following program standards and timelines, and involve family, significant others, and participant-identified collateral contacts in service provision.
- Collaborate with team members to provide coverage for after-hours reporting schedule to support participants engaged in employment, vocational training, and education services.
- Provide culturally competent services in accordance with the Center for Alternative's policies and practice.
- Develop and maintain strategic alliances with community providers.
Requirements:
- Master's degree in Social Work (MSW), Mental Health Counseling (MHC-LP), or a related field.
- Minimum of two years of experience working with individuals with mental illness, substance use disorders, co-occurring mental health and substance use disorders, and involvement in the criminal justice system.
- Experience understanding various models and theories pertaining to treatment of mental illness and substance use.
- Willingness to learn and adopt evidence-based principles to reduce recidivism risks.
- At least one year of experience conducting group sessions using evidence-based practice.
- Extensive knowledge of community treatment, support services, and resources.
- Highly organized and self-motivated.
Preferred Qualifications:
- Fluency in Spanish.
