Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Safe Haven Family Shelter. As a Case Manager, you will work closely with families experiencing homelessness to provide holistic and comprehensive support services.

Key Responsibilities
  • Conduct intake assessments and benefit assessments to identify client needs
  • Develop and implement individualized case plans to ensure successful transition to permanent housing
  • Provide case management services, including goal setting, progress monitoring, and advocacy
  • Connect clients with community resources and follow up to ensure successful referrals
  • Input accurate and complete data into agency HMIS and Salesforce database
  • Provide documentation and reports as required
Requirements
  • Bachelor's degree in Social Work or related field required; Master's degree preferred
  • Knowledge of community resources and counseling/social work practices with high-risk populations
  • Excellent verbal and written communication skills
  • Ability to work independently and as part of a team
  • Valid driver's license and ability to drive agency vehicles
