Job Summary

The Case Manager will provide counseling, advocacy, and case management services to domestic violence survivors within the Domestic Violence Services program. This includes coordinating in-house and community resources to meet the needs of clients. The Case Manager will conduct community outreach as needed on behalf of the YWCA.

Responsibilities
  • Identify barriers to client success and make appropriate referrals.
  • Provide individual and group counseling for residents living in Weaver Center, transitional housing clients, and community clients.
  • Assess client needs and develop case plans with residents, providing regular sessions to ensure progress.
  • Measure results, track data, and submit timely reports to the Program Manager.
  • Facilitate support groups with survivors of domestic violence.
  • Maintain accurate and confidential records for each client, documenting resident behaviors, incidents, interactions, and progress in a consistent and ethical manner.
Requirements
  • Bachelor's degree in Social Work or related field.
  • 1 to 3 years of case management experience (preferred), or 3 to 5 years of experience in a related field.
  • Valid driver's license, vehicle insurance, and a clean driving record for the last three years.
  • Ability to work long and flexible hours, including weekends and evenings.
  • Ability to work a rotating on-call schedule that includes late nights and weekends.
