Key Responsibilities
  • Provide Clinical Services: Assess, diagnose, and treat individuals and families experiencing homelessness, working collaboratively with a multidisciplinary team to develop and implement individualized treatment plans.
  • Deliver Quality Care: Provide chronic, preventive, acute, and outreach services based on established standards of care and evidence-based guidelines, ensuring patient-centered, trauma-informed, and non-judgmental care.
  • Participate in Interdisciplinary Team: Actively participate in an integrated, multidisciplinary team to develop and implement individualized treatment plans, contributing to interdisciplinary case conferences and collaborating with colleagues to achieve shared goals.
  • Document Services: Accurately complete documentation of services in the electronic medical record in a timely manner, ensuring compliance with regulatory requirements.
  • Stay Current with Best Practices: Demonstrate a commitment to ongoing learning and professional development, staying up-to-date with the latest scientific and practice literature to inform high-quality patient care.
  • Communicate Effectively: Communicate in a timely, clear, compelling, and concise manner with patients, families, and colleagues, building strong relationships and fostering a culture of collaboration and respect.
  • Set Goals and Monitor Progress: Set clear, meaningful, and attainable goals and expectations, monitoring progress towards meeting those outcomes and making adjustments as needed.
  • Address Health Disparities: Work to identify and address health disparities, actively developing and promoting a racial equity lens in your practice and supporting the development of a racial equity lens in colleagues.
Requirements
  • Education and Training: Master's or Doctorate Degree in Nursing as an adult or family nurse practitioner, with current licensure as a registered nurse practitioner in Maryland, CDS and DEA licenses, and BLS CPR certification.
  • Experience: 3 years of direct clinical care experience in primary care, preferably working collaboratively in a multi-disciplinary clinical setting, with comfort working with individuals and families who are homeless and/or low-income.
  • Skills: Excellent interpersonal skills, commitment to self-assessment and integrating, giving, and receiving constructive feedback, demonstrated efficiency, organization, and good judgment in the use of one's time and the time of others, strong written and verbal communication skills, demonstrated proficiency with EMRs, ability to cope with interruptions, be flexible, and be a team player, flexible approach, working with several cross-disciplinary teams in a collaborative style, culturally sensitive working with staff and clients from diverse backgrounds, and ability to work with ill, disabled, emotionally upset, and sometimes hostile individuals.
Working Conditions and Physical Requirements
  • Must be willing and able to work in outreach sites.
