LPN Visit Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Care Visit Nurse. As a key member of our healthcare team, you will provide high-quality patient care in the comfort of patients' homes, working closely with our multidisciplinary team to deliver exceptional care and support.
Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians, and record clinical and progress notes.
- Coordinate patient care activities and services to ensure the highest quality of care.
- Work collaboratively with our healthcare team to develop and implement individualized care plans.
- Ensure accurate and timely documentation of patient visits and care activities.
- Participate in care coordination and case management activities to ensure seamless care transitions.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
