LPN Visit Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family, part of LHC Group. As a LPN, you will provide direct nursing care to patients in their homes, working under the supervision of a Registered Nurse. This is a great opportunity for a nurse who wants to build trusted relationships with patients and their families, while also enjoying flexibility and opportunities for career growth.
Responsibilities- Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice.
- Document patient visits thoroughly and completely, following regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Ensure confidentiality of patient health information, as per agency policy.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
