As a Home Care LPN, you will provide direct nursing care to patients in their homes, working under the supervision of a Registered Nurse. Your primary responsibilities will include providing high-quality clinical services, documenting patient visits, and participating in the agency's care coordination process.
Key Responsibilities:- Provide direct nursing care to patients in their homes, following the plan of care and applicable infection control procedures.
- Document patient visits thoroughly and completely, adhering to regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Ensure confidentiality of patient health information, adhering to agency policy.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
