We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in La Crosse and surrounding areas. As a Visit Nurse LPN, you will provide high-quality clinical services to patients in their homes, working collaboratively with our healthcare team to deliver exceptional patient care.
Key Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians, and record clinical and progress notes.
- Coordinate patient care activities and services to ensure the highest quality of care.
- Work collaboratively with the healthcare team to develop and implement patient care plans.
- Ensure confidentiality of patient health information and maintain accurate records.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
