Responsibilities will encompass a wide array of health service delivery to both adults and children as a healthcare provider, within the nursing practice scope.

This role involves direct patient care responsibilities, including the assessment, diagnosis, and treatment of prenatal clients, as well as individuals seeking family planning, STD services, and preventive care.

This will involve reviewing previous medical records, reports, test results, and examination findings.
Rotates as necessary through various clinics, including adult health, child health, family planning, and communicable disease.

Given the dynamic nature of the work environment, staff roles and responsibilities may evolve as circumstances change.


  • Offers consultations and support to registered nurses operating in the community health department clinics.
  • Conducts physical examinations, medical diagnoses, and prescribes medications, as well as recommending dietary and activity programs based on the diagnosis.
  • Prescribes or administers treatments, therapies, medications, vaccinations, and other specialized medical care to treat or prevent illnesses, diseases, or injuries.
  • Conducts presentations on various topics as requested by the Executive Nursing Director.
  • Refers patients to medical specialists or other healthcare providers when necessary.
  • Ensures clear documentation in the electronic health record, with occasional paper formats expected. Accurate CPT and diagnosis coding must be performed for each patient visit, with documentation supporting this coding.
  • Provides input and reviews local health department protocols and assists in quality control.
  • Aids in the design and implementation of health promotion initiatives.
  • Participates in assessing the health needs of the community and in planning and revising programs to meet these needs.
  • Updates clinical knowledge and skills as required by state re-licensure and dictated by clinic rotations.
  • Assists in developing demographic data regarding community health to assess needs; maintains the quality of services delivered in all nursing practice tasks.

Provision of RN clinic support, as needed based on agency requirements: Provides direct patient care at Department of Health clinics, as assigned to support Immunizations, Family Planning, Preventive Care, TB, STD, and PrEP Clinics.


  • Conducts nursing assessments, medical histories, health promotion education, and wellness screenings.
  • Screens for domestic violence and provides intervention as necessary. Conducts HIV screening and counseling.
  • Performs screening and testing for tuberculosis.
  • Documents in the electronic health record and codes all services into the health management system.

ADMINISTRATIVE/DOCUMENTATION/TRAINING RESPONSIBILITIES:

  • Must uphold the quality of services delivered in all tasks associated with the position.
  • All information contained in health records is confidential, with access governed by state and federal laws. Confidential information includes the client's name, address, medical, social, and financial data, and services received. Attends and participates in staff meetings, in-service training, and other professional development activities.
  • Confidentiality- Ensure all information held in health records remains confidential, governed by state and federal laws during assigned projects. Will maintain confidentiality of information including the client's name, address, medical, social, and financial data, and services rendered.
  • Employees will respond to health alert network notifications within the required timeframe.
Knowledge, Skills, and Abilities Required:

  • Proficient knowledge of nursing principles, practices, and techniques.
  • Basic computer skills are essential.
  • Understanding of symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
  • Ability to review data for accuracy and completeness.
  • Ability to provide preventive health care treatment through teaching and demonstration.
