Job Title: Licensed Professional Clinical Counselor

Job Summary:
GuideStar Eldercare is seeking a highly skilled Licensed Professional Clinical Counselor to provide neurobehavioral health services to residents in long-term care facilities. As a key member of our clinical team, you will work closely with specialty clinicians to provide holistic, continuity of care for elderly individuals with neurocognitive disorders and other neurologic and/or psychiatric conditions.

Key Responsibilities:
• Provide neurobehavioral health services to residents in long-term care facilities
• Partner with specialty clinicians to provide holistic, continuity of care
• Utilize a multi-disciplinary approach to address the complex needs of elderly individuals
• Collaborate with the clinical team to develop and implement individualized care plans

Qualifications:
• Master's degree in Counseling
• Currently licensed as an LPCC in the state in which you are applying
