Job Title: Registered Nurse

We are seeking a skilled Registered Nurse to join our team at Element Care. As a key member of our PACE Interdisciplinary Team, you will play a vital role in delivering high-quality care to our participants.

Responsibilities:
  • Provide direct care to participants, including initial and periodic assessments, developing and revising care plans, and ensuring comprehensive decisions are made to ensure the safety and well-being of participants.
  • Collect and analyze medical and social information to inform care decisions and ensure conformance with established quality assurance and operational policies, procedures, and standards.
  • Monitor clinic care and service delivery, identifying areas for improvement and implementing effective solutions.
  • Coordinate with the interdisciplinary team to develop comprehensive care plans for each participant, incorporating immediate and continuing care needs.
  • Actively participate in implementing a summary on each client to help the state determine Nursing Home Level of Care.
  • Ensure all tests, lab work, and diagnostic studies ordered by the Physician have been carried out.
  • Coordinate all medication, including stocking, ordering, administering, and monitoring participant compliance.
  • Provide education and consultation to participants and caregivers on care plans, nursing treatments, and medications.
  • Communicate participant conditions to primary caregivers, physicians, and the multidisciplinary team.
  • Build strong relationships with participants, caregivers, families, and community providers to facilitate optimal care and financial oversight.
  • Participate in after-hour visits and maintain a high level of professionalism and respect in all interactions.
Requirements:
  • Registered Nurse currently licensed in the Commonwealth of MA.
  • Minimum of two (2) years experience with the geriatric population.
  • Proficient in the use of Microsoft Office tools.
  • Ability to travel locally on a frequent basis in a personal vehicle.
  • Ability to quickly recognize situations/problems before they become acute and implement effective solutions.
  • Covid vaccine required.
