Clinical Pharmacist

The Clinical Pharmacist position at the Veterans Affairs, Veterans Health Administration is a critical role that requires a licensed pharmacist with expertise in medication management and patient care. The ideal candidate will have a strong background in pharmacy practice, including pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.
Key Responsibilities- Provide clinical guidance on medication therapy to healthcare teams
- Conduct medication reconciliation and review patient medication lists
- Develop and implement medication therapy plans to optimize patient outcomes
- Collaborate with healthcare providers to resolve medication-related issues
- Provide education and counseling to patients and caregivers on medication use and management
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy
- Current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia
- 1 year of experience equivalent to the next lower grade level (GS-11) or completion of an ACPE-accredited Pharm.D. program
The Clinical Pharmacist will work in a fast-paced healthcare environment, interacting with patients, healthcare providers, and other staff members. The ideal candidate will be able to work effectively in a team setting, communicate complex information clearly, and maintain accurate records.
