Clinical Pharmacist

The Clinical Pharmacist is a licensed professional with expertise in pharmacy practice, including pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics. The incumbent is responsible for analyzing real and potential drug-related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy.
Key Responsibilities- Provides appropriate selection of drug therapy based on pharmaceutical principles of pharmacokinetics and pharmacodynamics.
- Monitors for efficacy, side effects, and clinical outcome, and advises prescribers as appropriate.
- Evaluates and assesses prescribed medication to assure optimal drug therapy, and addresses/discontinues duplicate therapies/prescriptions that are no longer required/desired (poly-pharmacy).
- Provides medication counseling to patients, family members, and caregivers as needed/appropriate.
- Participates in reviewing charts, evaluating pertinent laboratory data, drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies.
- Participates in medical emergencies, including code teams and emergency preparedness activities as requested.

- Must be proficient in written and spoken English.
- Physical Requirements: Work is primarily sedentary, but requires extreme patience and controlling one's emotions.
- You may be required to serve a probationary period.
