Inpatient Care Coordinator

MaineGeneral Health is seeking a skilled Inpatient Care Manager to optimize the quality and continuity of care for patients during their hospital stay. As a key member of our healthcare team, you will promote positive health outcomes and patient satisfaction.
Job Description:The Role:
Inpatient Care Manager (RN or LSW)
Key Responsibilities:
- Investigate, assess, and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
- Consult with multidisciplinary teams regularly to evaluate patient's status and appropriateness of medical care.
- Participate in discharge planning, including identification and coordination of appropriate resources.
- Provide discharge education and resource referrals to patients when necessary.
Requirements:
- Maine state licensure as a Licensed Social Worker (LSW), or Registered Nurse (RN) is required.
- Excellent teamwork, communication, and collaboration skills.
- Proven and demonstrated critical thinking skills.
