Job Summary:

Job Description:

The Role:

Inpatient Care Manager (RN or LSW)

Key Responsibilities:

  • Investigate, assess, and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Consult with multidisciplinary teams regularly to evaluate patient's status and appropriateness of medical care.
  • Participate in discharge planning, including identification and coordination of appropriate resources.
  • Provide discharge education and resource referrals to patients when necessary.

Requirements:

  • Maine state licensure as a Licensed Social Worker (LSW), or Registered Nurse (RN) is required.
  • Excellent teamwork, communication, and collaboration skills.
  • Proven and demonstrated critical thinking skills.
