Job Overview:

Compensation: This position offers a competitive salary range of approximately $90,000 - $125,000 per year, based on your level of experience and expertise.

Key Responsibilities:
- Assess patient needs, develop, and implement individualized care plans to achieve optimal patient outcomes.
- Educate patients and their families on various aspects of health, wellness, and disease management, empowering them to take control of their health.
- Foster collaborative relationships with multidisciplinary teams to ensure seamless care delivery and improve overall patient satisfaction.
- Actively participate in quality enhancement initiatives to drive continuous improvement in patient care and service excellence.

Qualifications:
- Possess a valid New York State RN license and Bachelor's degree in Nursing (BSN).
- Bring at least 2 years of experience in a similar nursing role, demonstrating your ability to excel in fast-paced environments and lead by example.
- Showcase proficiency in electronic medical records systems and basic computer applications.
