About the Role:

We are seeking a skilled and compassionate Traveling Nurse Practitioner to join our team as a HouseCalls Specialist. As a key member of our healthcare team, you will conduct in-home assessments, provide comprehensive physical exams, and develop personalized care plans for patients across Texas.

Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate)
  • Identify diagnoses and formulate treatment plans using clinical knowledge and judgment
  • Communicate findings with patients' primary care physicians to inform care management
  • Educate patients on disease processes, medication, and compliance

Requirements:

  • Active NP license or ability to obtain by start date; licensure must be unencumbered
  • ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain within 6 months
  • Ability to complete physical requirements of the job, including lifting up to 30 pounds and navigating stairs
  • Access to reliable transportation for travel across Texas (up to 85% of work time)
