Position Overview: Service Coordinator
Type: Full-Time
Location: East Patchogue, NY
Salary: $55,000
Schedule: Monday to Friday; 1 PM - 9 PM

Role Responsibilities:
The Service Coordinator plays a vital role in connecting clients with community resources and providing essential support services. Key responsibilities include:

  • Facilitating access to community services such as case management, clinical support, vocational opportunities, and entitlement programs.
  • Developing quarterly goal plans and conducting functional assessments.
  • Conducting weekly meetings with clients to enhance skills and monitor progress.
  • Maintaining accurate documentation for client records, including progress notes and discharge plans.
  • Offering individual counseling and support tailored to client needs.
  • Assisting with housing-related maintenance issues.
  • Providing training in daily living skills, including medication management and financial literacy.
  • Conducting monthly fire safety inspections.
  • Participating in weekly supervision and staff meetings.
  • Staying updated with all compliance requirements and training.
  • Performing additional duties as assigned by the Program Director or Supervisor.

Qualifications:
Applicants should possess a Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health field, along with a minimum of two years of experience in a similar role or working with individuals with mental health challenges. Strong verbal and written communication skills are essential, as well as the ability to make informed decisions regarding client care.
