We operate services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

Salary and Benefits

The estimated salary for this role is $24.98 - $30.17 per hour, based on the location and job requirements. In addition to a competitive salary, we offer our employees a range of benefits, including medical, dental, and vision coverage, vacation and sick time, paid holidays, and a retirement plan.

About the Case Manager Role

The Case Manager plays a crucial role in assisting clients in breaking the cycle of homelessness. This position supports clients through all phases of homelessness, from on-the-street services to transitional and permanent housing.

  • Conducts intake and individualized needs assessments for clients, developing Individualized Service Plans that address housing goals, income, and other personal objectives.
  • Provides referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
  • Completes program-specific assessments, such as the VI-SPDAT.
  • Evaluates client strengths and challenges, conducting 90-day reviews with clients after initial intake.

What You Bring

We're looking for candidates with a strong background in healthcare, case management, homeless services, or working with vulnerable populations. A minimum of two years of experience is preferred, but all levels of education and experience are welcome.

In addition to your qualifications, you must have reliable transportation, a valid driver's license, proof of insurance and ownership for your vehicle, and the ability to qualify for PATH's insurance coverage.
