Transforming Lives through Empathy and Support
As a Peer Navigator, you will work closely with our team to provide personalized support and guidance to patients. Your empathetic approach and passion for making a difference will help create positive outcomes and improve overall quality of life.
About the Role
- Build strong relationships with patients by sharing your personal experiences and insights.
- Craft and implement tailored strategies to address common health concerns and promote healthy behaviors.
- Connect patients with local resources and services to overcome barriers to care and enhance treatment adherence.
- Foster a sense of community by facilitating connections between patients and providing ongoing support.
- Act as a patient advocate, advocating for their needs and ensuring they receive necessary medical attention and supplies.
- Conduct regular outreach and follow-ups to ensure patients receive comprehensive care and support.
- Develop and maintain effective working relationships with colleagues from various departments to ensure seamless care coordination.
Your Qualifications
- A minimum of 3+ years of experience in direct patient care or related fields, preferably working with homeless populations.
- Bachelor's degree preferred, but not required, along with relevant work experience or training.
- Ability to communicate effectively, articulating your own experiences and connecting with patients on a personal level.
- Strong organizational skills, enabling you to prioritize tasks, manage multiple responsibilities, and maintain accurate records.
- Ability to work independently and collaboratively as part of a dynamic team.
- Proficiency in operating computer systems, including electronic health records, call center software, and HMIS/Coordinated Entry System (preferably).
- Maintain a valid driver's license, clean driving record, automobile insurance, and reliable transportation for frequent travel.
