Key Responsibilities:
  • Conduct comprehensive client assessments and develop personalized service plans to meet client needs.
  • Provide advocacy, case management, and supportive counseling to individuals, families, and groups.
  • Collaborate with community resources to ensure clients receive the necessary support and services.
  • Handle emergency and crisis situations with sensitivity and professionalism.
  • Monitor client progress and adjust service plans as needed.
Requirements:
  • Bachelor's degree in Social Work or a related field from an accredited college or university.
  • One year of case management experience in a social service or healthcare setting.
  • Crisis stabilization experience preferred.
  • Bilingual (English/Spanish) preferred.
  • Valid driver's license and proof of insurance required.
