Job Title: Community Support Specialist

Concordance is seeking a highly motivated and compassionate Community Support Specialist to join our team. As a key member of our reentry program, you will play a vital role in supporting individuals reentering the community from incarceration.

Job Summary:

The Community Support Specialist will provide transportation and support to participants, ensuring they have access to essential resources and services. This includes coordinating transportation to medical, dental, and psychiatric visits, as well as helping participants obtain identification documents and access to food stamps and Medicaid insurance.

Key Responsibilities:
  • Provide transportation and support to participants, building rapport and trust through a trauma-informed approach.
  • Review daily schedules to coordinate participant transportation needs and communicate with the Center Coordinator on any schedule changes.
  • Transport participants to scheduled appointments, AA/NA meetings, and part-time employment.
  • Ensure participants have necessary documentation and funding for appointments.
  • Collaborate with Concordance team members to document transports and maintain a positive, mission-focused work environment.
Requirements:
  • Valid Class E Driver's License and comfortability transporting groups in a vehicle provided by Concordance.
  • High school degree or equivalent.
  • 2+ years of experience in social service or transportation field preferred.
  • CPR/Adult First Aid certification preferred.
