Job Description:

Broadstep Behavioral Health is seeking a dedicated Care Manager to join our Tailored Care Management (TCM) team. This role is essential in delivering comprehensive care management services within the framework of NC Medicaid Managed Care, emphasizing a holistic approach to health and wellness.

Job Type: This position is full-time and requires onsite presence.

Schedule: Monday to Friday, 8am-5pm (with some flexibility)

Job Responsibilities:

  • Create comprehensive assessments and individualized care plans for beneficiaries.
  • Coordinate and monitor a range of services including medical, pharmacy, behavioral health, and community resources.
  • Provide crisis support and planning for beneficiaries in need.
  • Facilitate annual physical examinations.
  • Oversee care coordination for Innovations and TBI waiver services, if applicable.
  • Collaborate with a multidisciplinary care team to ensure effective service delivery.
  • Manage care for beneficiaries with chronic and high-risk conditions.
  • Assist with medication management and monitoring.
  • Identify and address unmet health-related resource needs.
  • Educate beneficiaries on chronic health conditions and promote self-management strategies.
  • Monitor Hospital Admission Discharge and Transfer (ADT) alerts to ensure appropriate follow-up.
  • Support beneficiaries during transitions from hospitals and nursing facilities.

Job Requirements:

  • A bachelor's degree in health-related fields such as psychology, sociology, social work, nursing, or a relevant human services area, or licensure as a registered nurse (RN).
  • A minimum of two years of experience working directly with individuals facing behavioral health challenges or those with I/DD or TBI conditions.
  • For care managers serving members with LTSS needs: two years of experience in LTSS and/or HCBS coordination and care management, in addition to the above qualifications.
