With over 45 years of experience, Broadstep has established itself as a pioneer in delivering a wide range of physical, emotional, and mental health support services for individuals with intellectual and developmental disabilities (I/DD), mental health challenges, and co-occurring disorders.
Our North Carolina division is committed to community-based mental health and substance abuse programs that prioritize the concept of whole-person care and wellness. We proudly hold CARF Accreditation.
Job Type: This position is full-time and requires onsite presence.
Schedule: Monday to Friday, 8 AM to 5 PM (with some flexibility)
Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans with beneficiaries.
- Coordinate and monitor a variety of services, including medical, pharmacy, behavioral health, and community resources.
- Provide crisis support and planning assistance to beneficiaries.
- Facilitate annual physical examinations.
- Oversee care coordination for Innovations and TBI waiver services, as applicable.
- Collaborate with a multidisciplinary care team to ensure comprehensive support.
- Manage care for beneficiaries with chronic, high-risk, and high-cost needs.
- Assist with medication management and monitoring.
- Identify and address unmet health-related resource needs.
- Educate beneficiaries on chronic health conditions and promote self-management strategies.
- Monitor Hospital Admission Discharge and Transfer (ADT) alerts to ensure appropriate follow-up.
- Support beneficiaries during transitions from hospitals and nursing facilities.
Qualifications:
- A bachelor's degree in health-related fields such as psychology, sociology, social work, nursing, or a similar area, or licensure as a registered nurse (RN).
- A minimum of two years of experience working directly with individuals experiencing behavioral health conditions or those with I/DD or TBI.
- For care managers assisting members with LTSS needs: two years of experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is required, in addition to the qualifications mentioned above.
