Job Type: This position is full-time and requires onsite presence.

Schedule: Monday to Friday, 8 AM to 5 PM (with some flexibility)

Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans with beneficiaries.
  • Coordinate and monitor a variety of services, including medical, pharmacy, behavioral health, and community resources.
  • Provide crisis support and planning assistance to beneficiaries.
  • Facilitate annual physical examinations.
  • Oversee care coordination for Innovations and TBI waiver services, as applicable.
  • Collaborate with a multidisciplinary care team to ensure comprehensive support.
  • Manage care for beneficiaries with chronic, high-risk, and high-cost needs.
  • Assist with medication management and monitoring.
  • Identify and address unmet health-related resource needs.
  • Educate beneficiaries on chronic health conditions and promote self-management strategies.
  • Monitor Hospital Admission Discharge and Transfer (ADT) alerts to ensure appropriate follow-up.
  • Support beneficiaries during transitions from hospitals and nursing facilities.

Qualifications:

  • A bachelor's degree in health-related fields such as psychology, sociology, social work, nursing, or a similar area, or licensure as a registered nurse (RN).
  • A minimum of two years of experience working directly with individuals experiencing behavioral health conditions or those with I/DD or TBI.
  • For care managers assisting members with LTSS needs: two years of experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management is required, in addition to the qualifications mentioned above.
