Role Overview:
The Clinical Care Coordinator plays a crucial role in managing care for pediatric patients facing rehabilitation challenges or dealing with acute, chronic, or life-altering health conditions.

This position acts as a vital link among patients, families, healthcare providers, therapists, admissions personnel, administrative staff, insurance representatives, and community service organizations, all within a family-centered care framework.


Key Qualifications:
Mandatory:
- Bachelor’s degree with a background in care coordination.
- Experience working in pediatric healthcare settings.

Core Responsibilities:

- Facilitates comprehensive care for infants, children, and young adults throughout various healthcare stages, including intake, outpatient, inpatient, and long-term care services.

- Ensures effective communication between patients, families, and both internal and external service providers.
- Collaborates as an essential member of a multidisciplinary healthcare team.

- Demonstrates knowledge of family-centered care principles, understanding of medical diagnoses, prognoses, and implications for family support and education.

- Provides guidance for families seeking services to better manage chronic illnesses and assesses their service needs.
- Organizes and reviews intake documentation, consulting with team leaders as necessary.
- Exhibits a solid understanding of the healthcare system, managed care, and resource management.
- Coordinates scheduling for evaluations and treatments for prospective patients.

- Supports the navigation process for inpatient rehabilitation and long-term care by collaborating with key stakeholders and multidisciplinary teams.

- Collects, manages, and documents all relevant patient information.

- In alignment with RWJBarnabas Health's mission and values, advocates for the needs of patients and families across medical, social, and financial dimensions.

- Provides necessary documentation to external agencies as required.
- Additional duties and responsibilities may evolve over time.
