**Job Summary**

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Addington Place of Stuart. As an LPN Assisted Living Specialist, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.

**Key Responsibilities:**

  • Administer medications to residents in accordance with federal and state regulations, as well as Senior Lifestyle Corporation guidelines.
  • Conduct admissions assessments and documentation for new residents, ensuring a smooth transition into our community.
  • Develop and implement acute and long-term care plans or service plans, tailored to each resident's unique needs.
  • Monitor and track clinical systems, such as wound care and infection control, under the direction of the director.
  • Provide health-related consultations to residents and staff, promoting a culture of wellness and prevention.
  • Notify physicians and family members of any changes in a resident's health, ensuring timely and effective communication.
  • Respond promptly to emergencies, prioritizing the safety and well-being of our residents.
  • Maintain open communication with all departments and staff members, fostering a collaborative and supportive environment.
  • Develop and lead educational meetings for residents and staff, promoting knowledge sharing and skill development.
  • Ensure confidentiality of resident and prospective resident information, upholding the highest standards of professionalism and integrity.
  • Assist with discharges, assessments, and clinical documentation as needed, ensuring seamless transitions and accurate records.

**Requirements:**

  • Training from an accredited licensed or vocational nursing program
  • General nursing area of study
  • 1+ years of experience in senior care or a related field
  • Current Licensed Practical Nurse's License in the state of practice
  • CPR certification
  • Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond to emergency situations
  • Basic computer skills
  • Ability to work as part of a team and handle multiple tasks safely and effectively
