About the Role
  • Deliver outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties.
  • Collaborate with an interdisciplinary team to perform psychosocial evaluations and assess mental health service needs of clients and their families.
  • Create and execute short-term service plans for clients, ensuring seamless program operations and vital liaison to community agencies.
  • Respond promptly to dispatch calls, traveling from the office to clients' homes within two hours, and ensure uninterrupted comprehensive care.
  • Prepare case histories and maintain case records, in accordance with the Program record-keeping mechanism.
  • Encourage resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare.
  • May provide clinical supervision for Mental Health Technician, graduate Social Worker students, or junior staff Social Workers.
Requirements
  • Current registration to practice as a Licensed Master Social Worker in New York State.
  • Valid driver's license may be required.
  • Minimum of one year experience as a Social Worker in a healthcare setting.
  • Master's degree in social work from a graduate school of Social Work accredited by the Council on Social Work Education and the Education Dept.
