Reports to: Program Supervisor for Care Coordination

Job Purpose:

The primary role of the Bilingual Care Coordination Specialist is to deliver comprehensive management services to individuals enrolled in the Care Coordination Program, focusing on those affected by HIV and/or mental health challenges.

In this position, you will be expected to foster an environment that aligns with the organization's Cultural Equity Policy and the principles of WELCOME Orange.

Key Responsibilities:

  • Participate in bi-weekly team meetings dedicated to Care Coordination.
  • Engage in training sessions and meetings pertinent to Health Homes and Care Coordination.
  • Provide care coordination services as specified in the Care Coordination Model.
  • Conduct New York State Community Mental Health (CMH) assessments and follow-up evaluations.
  • Process monthly billing for your assigned caseload.
  • Compile monthly scorecard data collection metrics.
  • Maintain electronic client records using Foothold Technology and GSI Electronic Record Keeping systems.
  • Ensure all client files are managed in accordance with program policies and procedures.
  • Establish and sustain connections with relevant community organizations and service providers.
  • Represent the organization at various meetings as required.

Additional Responsibilities:

  • Assist in answering the main office phone as needed, promoting teamwork.
  • Contribute to organizational committees as necessary.

Qualifications:

Education:

A Bachelor's Degree in fields such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR New York State licensure and current registration as a Registered Nurse with a bachelor's degree; OR A bachelor's degree in any discipline with five years of experience working directly with individuals with behavioral health diagnoses; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).

Experience:

A minimum of two years of experience (a Master's degree in a related field may substitute for one year of experience) in:

  • Providing direct services to individuals diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse; OR
  • Connecting individuals diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse to a wide range of essential community services.

Bilingual proficiency in Spanish and English is required.

Strong written and verbal communication skills are essential.

Experience with Health Homes is highly preferred.

Familiarity with Medicaid procedures is highly preferred.

Experience with Electronic Health Record systems is mandatory.

Proficiency in Microsoft Excel and Word is preferred.

Ability to work independently with minimal supervision is necessary.

Must demonstrate strong organizational skills, self-motivation, and the capacity to manage multiple tasks simultaneously.

Ability to exercise sound judgment in crisis situations and adhere to confidentiality regulations is crucial.

A valid Driver's License and the ability to drive with an acceptable driving record for agency insurance coverage are required.
