Job Title: Hospice Aide Specialist

We are seeking a compassionate and skilled Hospice Aide Specialist to join our team at Hospice of Orange & Sullivan Counties. As a key member of our interdisciplinary team, you will provide high-quality care and support to patients and their families in their homes and communities.

Job Summary:

The Hospice Aide Specialist will be responsible for providing personal care and assistance to patients with terminal illnesses, as well as supporting their families and caregivers. This role requires a strong understanding of hospice care principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Provide personal care and assistance to patients, including bathing, dressing, and grooming.
  • Assist patients with daily living activities, such as eating, toileting, and transferring.
  • Support patients and their families with emotional and spiritual needs.
  • Document patient care and progress in electronic medical records.
  • Communicate effectively with patients, families, and healthcare team members.
  • Participate in interdisciplinary team meetings and contribute to patient care planning.
  • Maintain confidentiality and adhere to HIPAA guidelines.
Requirements:
  • One year of experience in a hospital or nursing home setting (preferred).
  • Successful completion of recognized HHA Certification program required.
  • At least eighteen years of age and high school graduate or equivalent.
  • Ability to read and follow written instructions and document care given.
  • Displays initiative and able to work with minimal direct supervision.
  • Has empathy for the needs of the ill, frail, and impaired.
Work Environment:

This role requires working in patients' homes and communities, with occasional travel to our office. You must have a reliable vehicle and valid driver's license. You will be required to lift, position, and transfer patients, as well as lift supplies and equipment.
