Job Title: Licensed Practical Nurse - LPN

We are seeking a skilled and compassionate Licensed Practical Nurse - LPN to join our team at Ancora Memory Support at Mount Alverna Village, a part of Franciscan Ministries. As an LPN, you will play a vital role in providing high-quality care to our residents and contributing to a supportive and nurturing environment.

Responsibilities:
  • Provide direct nursing care to residents, including administering medications, wound care, and vital sign monitoring.
  • Supervise and coordinate the activities of Resident Care Assistants (RCAs) to ensure a high level of quality care is maintained.
  • Responsible for admission, transfer, and discharge of residents, ensuring a smooth transition and minimizing disruptions.
  • Keep physicians and other healthcare professionals informed of changes in resident conditions, promoting effective communication and collaboration.
  • Perform specific clinical skills necessary for the nursing care of residents, including vital sign monitoring, medication administration, and wound care.
  • Participate in the documentation of nursing care, ensuring accurate and timely records.
Requirements:
  • Graduate of an accredited LPN program.
  • LPN license in Ohio.
  • Recent clinical experience, education, and specialty skills specific to geriatrics.
  • Flexible schedule, including availability to work evenings, weekends, and holidays as needed.
  • Must enjoy working with the senior population.
