Job Title: Licensed Practical Nurse - LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Ancora Memory Support at Mount Alverna Village, a vibrant senior living community in Parma, Ohio.

About the Role:

As an LPN, you will play a vital role in providing direct nursing care to our residents, supervising and coordinating activities, and ensuring a high degree of quality care is maintained. You will also be responsible for admission, transfer, and discharge of residents, keeping physician and/or other health care professionals informed of changes in resident conditions.

Responsibilities:
  • Provide direct nursing care to residents, including administering medications, performing clinical skills, and participating in documentation of nursing care.
  • Supervise and coordinate activities performed by Resident Care Assistants (RCAs) to ensure a high degree of quality care is maintained.
  • Responsible for admission, transfer, and discharge of residents, ensuring a smooth transition and maintaining accurate records.
  • Keep physician and/or other health care professionals informed of changes in resident conditions, ensuring timely and effective communication.
Requirements:
  • Graduate of an accredited LPN program.
  • LPN license in Ohio.
  • Recent clinical experience, education, and specialty skills specific to geriatrics.
  • Flexible schedule, including availability to work evenings, weekends, and holidays as needed.
