Job Description

The Hope to Health Clinic, a unique and innovative health clinic in the Downtown Eastside, is seeking a Licensed Practical Nurse to join its multidisciplinary clinical team. As a key member of the team, you will provide primary and specialty care to patients/clients in the Downtown Eastside, working collaboratively with physicians, nurses, counsellors, and peer navigators to ensure comprehensive care.

Responsibilities
  • Provide safe and respectful nursing care to an assigned group of patients/residents, according to established policies and procedures.
  • Conduct head-to-toe assessments, monitor and recognize abnormal signs or symptoms, and communicate with the RN/RPN as required.
  • Communicate with physicians regarding assigned patients/residents and carry out verbal and/or written orders as required.
  • Document care provided, including assessments, status, progress, treatments, changes in condition, medications administered, and discharge planning.
  • Administer medications, including narcotics, according to established procedures, and monitor medication effects.
  • Respond to incidents such as Codes, injuries or falls, hazardous spills, aggressive behavior, and fire alarms, reporting incidents and filling out appropriate forms in accordance with established procedures.
  • Assist with the orientation of new staff and student preceptors by performing duties such as demonstrating work procedures, proper use/location of equipment, sharing clinical knowledge, and acting as a positive role model.
Requirements
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
  • One-year recent, related experience working in an urban health setting, including experience with addiction, HIV/AIDS, or indigenous patient populations.
