Empowerment Case Manager

Job Title: Empowerment Case Manager
Job Summary:
The Empowerment Case Manager position is responsible for implementing the Empowerment and Direct Assistance Program, including client selection, enrollment, follow-up, and pre- and post-measurement assessments and outcome measures.
Key Responsibilities:
- Develop and maintain partnerships with community agencies
- Meet with clients to determine admissibility and provide orientation
- Conduct needs assessments and collaborate with clients to write individualized service plans
- Work with clients to meet the goals of their individualized plans
- Provide ongoing case management and follow-up
- Refer clients to outside community resources when needs cannot be met
- Input all required information into Client Track and ensure adequate and accurate documentation
- Evaluate success of clients
- Advocate on behalf of clients and the organization
- Complete and submit accurate and timely reports
Requirements:
- Bilingual (English/Spanish)
- Minimum of a two-year college degree in human services or related field
- 2 years of successful experience in working in human service field or related field
- Experience in establishing and maintaining records, reports, and other data
- Demonstrates initiative, self-motivation, and strong organizational skills
- Demonstrates warmth, compassion, empathy, honesty, and maturity
- Ability to respond appropriately and with sensitivity to the cultural differences among our client populations
- Must be proficient in basic office software such as Word, Excel, and Outlook
- Ability to create computer-driven reports
- Willingness to work within the framework of Catholic social teachings
