We are seeking a skilled Care Coordinator to join our team at Trinity Health Of New England. This critical role involves providing patient-centered experiences through scholarly scientific inquiry, compassion, advocacy, and education.

Job Overview

This position is located in an outpatient clinic office environment, servicing the following service lines: Thoracic Surgery, Interventional Pulmonology, Lung Screening Program, Firefighter Cancer Screening Program, and Incidental Nodule (FIND) Program.

Key Responsibilities:
  • Acts as the single point of contact for all thoracic patients.
  • Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements.
  • Utilizes evidence-based practice or research within clinical practice.
Requirements

To be successful in this role, you will need:

  • Bachelor's degree in nursing or Associate's degree in nursing with a minimum of five years of nursing experience.
  • Current licensure as a registered nurse (RN) in Massachusetts.
  • CPR certification.
  • Three to five years' nursing experience.
Estimated Salary Range

The estimated salary range for this role is between $80,000 and $110,000 per year, depending on experience and qualifications.
