We are seeking a highly experienced and skilled Director of Patient Care Services - Home Care to join our team at Riverscapesearch. As a key member of our leadership team, you will be responsible for overseeing the delivery of high-quality patient care services in the home setting.
Key Responsibilities- Develop and implement clinical policies and procedures to ensure compliance with regulatory requirements and industry standards.
- Lead and manage a team of healthcare professionals to provide exceptional patient care and services.
- Collaborate with cross-functional teams to ensure seamless delivery of patient care services.
- Monitor and evaluate clinical outcomes and performance metrics to identify areas for improvement.
- Develop and implement quality improvement initiatives to enhance patient care and services.
- Ensure compliance with regulatory requirements, including The Joint Commission standards and Medicare certification.
- Develop and maintain relationships with healthcare providers, payers, and other stakeholders to ensure optimal patient care and services.
- Stay up-to-date with industry trends, best practices, and regulatory changes to ensure Riverscapesearch remains a leader in home care services.
- Minimum three (3) years of supervisory/management experience in a licensed Medicare-certified home health or Hospice agency within the last seven (7) years.
- OR minimum five (5) years of home care consulting, accreditation supervisory/management experience in an inpatient, outpatient, or chief nurse role.
- Bachelor's degree in nursing or other healthcare-related field; Master's degree preferred.
- Registered Nurse License (California) required at hire.
- OASIS certification of home health or hospice or administrative certification, same subject.
- Ability to demonstrate knowledge of and apply state and federal regulations, including The Joint Commission standards and requirements regarding Home Care operations and services.
