Job Overview:
This full-time office position involves developing, implementing, and evaluating individualized patient care plans as a Registered Nurse case manager for First Option Home Health.
Salary and Benefits:
The estimated annual salary for this role is $80,000-$100,000, depending on experience, with opportunities for bonuses and competitive benefits package including health insurance, dental, life, disability, and other supplemental insurance policies.
Key Responsibilities:
- Function as primary RN team leader for a select group of patients
- Work with new admissions, set plan and goals from collection of data from our interdisciplinary team; referral sources
- Effectively learn and provide care under all insurance carriers; follow all regulatory policies and guidelines
- Constantly manage patient episode of care; manage care needs, scheduling; delegate services timely
- Regularly evaluate patient care plans and update them on an as-needed basis
- Proficient in company's EMR system; scheduling
- Participate in on-call rotation with other RN administrative staff monthly; lead case conferences weekly
- Able and willing to perform any additional duties related to patient care and compliance as directed by D.O.N. within scope of nursing license
Requirements:
- Must have an Registered Nurse license (Associate/BSN)
- Must have a minimum of 3 years recent home health (previous case management experience in home health preferred)
- Must show ability to function effectively and consistently as a Communicator and Coordinator to achieve both Clinical and Financial goals for a select group of patients
- Must be able to work calmly under a fast-paced work environment and able to manage time and deadlines for regulatory compliance and reimbursement for your group of patients
