Chronic Care Management Health Navigator

We are seeking a highly skilled Chronic Care Management Health Navigator to join our team at SWLA Center for Health Services. This role is responsible for coordinating care, educating patients, and building trust between patients and medical practitioners.
Key Responsibilities:- Assess cognitive/verbal skills and identify barriers to accessing healthcare.
- Provide individual and family educational interventions, including self-management goal setting, counseling, and training on habits, lifestyle changes, supplies, and tools necessary to manage chronic diseases effectively.
- Perform individualized assessments of patients' educational needs and provide tools to aid in managing their diseases effectively.
- Monitor patients for changes in health status after initiation of new medications, hospitalizations, or recent declines in function.
- Follow up with patients when barriers to referrals are identified.
- ADN, BSN, or Degree in Nursing required; 3-5 years' work experience related to chronic disease management and active licensure as an RN in Louisiana.
- Experience working in clinical/community out-patient settings and working with diverse population groups preferred.
- Content knowledge and expertise in program-specific fields required.
