About the Role

Job Summary

As a Patient Care Coordinator at Atrium Health, you will play a vital role in ensuring the efficient and high-quality care of our patients. This position will be responsible for coordinating the patient's care from the time of diagnosis through their entire care journey. You will serve as a patient advocate, providing personalized support and ensuring that patients and families receive the necessary resources and services.



Key Responsibilities

  • Provide language-specific education and psychosocial support to newly diagnosed patients and their families, and coordinate referrals for necessary services.
  • Educate patients and families about diagnosis, treatment options, and available resources, including financial assistance and community support.
  • Serve as a liaison between departments to ensure seamless communication and coordination of care.
  • Coordinate new patient referrals, scheduling of exams and appointments, and identify potential gaps in care.
  • Assess patients for emotional and social needs, and refer them to relevant resources.
  • Identify trends in barriers to patient care and implement service recovery strategies to improve patient satisfaction.

Requirements

Education, Experience, and Certifications

A BSN degree is required, with an MSN degree preferred. Current licensure as a Registered Nurse in North Carolina is required, and certification in a specialized field is required within two years of employment. Excellent communication skills are essential for this role.
