Job Title: Home Health Physical Therapist Assistant

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapist Assistant to join our team in Detroit, Michigan.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist Assistant to provide comprehensive skilled services to patients in their homes. The ideal candidate will have a strong background in physical therapy and excellent communication skills.

Responsibilities:
  • Provide skilled services to patients in their homes, including physical therapy procedures and diagnostic tests.
  • Observe, record, and report patient reactions, changes, and progress.
  • Communicate effectively with physicians, therapists, and coworkers to ensure seamless care.
  • Participate in case conferencing to promote coordination of care.
  • Recommend prosthetic/orthotic devices for patients and evaluate their use.
  • Educate patients and family members on therapy techniques, safety measures, and supportive activities.
  • Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current license as a Physical Therapist Assistant in the state of employment.
  • Minimum of one year of experience as a PTA in an acute setting.
  • Home care experience is desired.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians, and coworkers.
  • Knowledgeable on therapy best practices.
  • Ability to make appropriate physical therapy judgments.
  • Ability to identify a situation and handle it with the best possible solution.
  • Detail-oriented and observant.
  • Disciplined style of work ethic with the ability to prioritize and be timely.
  • Valid driver's license and auto insurance with your name as a listed driver.
Working Conditions:

The Physical Therapist Assistant will work in a home health setting, providing services to patients in their homes. The ideal candidate will be able to work independently and as part of a team to provide comprehensive care.
