Home Health Physical Therapist Assistant Job Description

We are seeking a skilled Physical Therapist Assistant to provide high-quality care to our patients in the comfort of their own homes. As a member of our team, you will work closely with our healthcare professionals to develop and implement personalized therapy plans that promote optimal health and well-being.

Responsibilities:
  • Deliver physical therapy services and education to patients in their homes, adhering to physician-ordered plans of care and therapy care plans.
  • Participate in interdisciplinary activities to ensure comprehensive patient care.
  • Perform duties in accordance with Agency policies, state, federal, and professional standards.
  • Assume additional responsibilities as needed to support the delivery of exceptional patient care.
Requirements:
  • Current licensure to practice as a Physical Therapist Assistant in the state(s) of employment.
  • Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association and CAPTE, with a passing score on the national examination for PT assistants.
  • Minimum of six (6) months of experience as a Physical Therapist Assistant.
  • Demonstrated strong written, verbal communication, and interpersonal skills.
  • Licensed driver with automobile insurance meeting state/Agency requirements, and reliable transportation with a good driving record.
